Chronic Renal Failure (CRF)


Chronic Kidney Disease (CKD), also known as chronic renal disease and commonly Kidney Failure, is a progressive loss in renal function over a period of months or years. The most common cause is Diabetic Nephropathy followed by Hypertensive Nephroangiosclerosis and various primary and secondary Glomerulopathies. More over Pain-killers are playing a big role. Obstructive Uropathy due to Nephrolithiasis and Prostatic Hypertophy are common, while congenital conditions such as Polycystic Kidney Disease (ADPKD) also lead to CKD. The normal Glomerular Filtration Rate (GFR) is normally 125 ml/min.

STAGE I – When GFR falls below 90 ml/min it is stage I of Chronic Kidney Disease. There are usually with no signs or symptoms at this stage.

STAGE II – When GFR falls at 30-70 cc/min it is stage II or Chronic renal insufficiency (CRI).
The symptoms at this stage are unspecific, and might include general feeling of being unwell and experiencing a reduced appetite. Often CRI is diagnosed as a result of screening of people known to be at risk of kidney problems, such as those with high Blood Pressure or Diabetes and those with a blood relative with Chronic kidney disease (CKD).

STAGE III - Chronic Renal Failure (CRF) is progressive destruction of renal mass with irreversible sclerosis and loss of nephrons over a period of at least months to many years with progressive decrease of Glomerular Filtration Rate (GFR) below 30cc/min.The symptoms are typically of worsening kidney function such as anorexia, nausea, vomiting, oliguria, fatigue and swelling.

STAGE IV - In End-Stage Renal Disease (ESRD) the Glomerular Filtration Rate (GFR) declines to levels of less than 10 cc/min with full blown signs and symptoms of Uraemia and fluid overload. It renders the patient permanently dependent upon Renal Replacement Therapy (RRT) i.e. Dialysis or Transplantation. Incidence of Kidney Failure in India is about 260 pmp, out of which 2 undergo Hemo-Dialysis (HD), 1 Continuous Ambulatory Peritoneal Dialysis (CAPD) and 3 Renal Transplant (RT), while 254 pmp are left to die every year, as they cannot afford the cost of treatment.
